Whats wrong with my rescued dog?

so i adopted a 5 year old dog, and she wont get off the couch and when she does she only goes to a blanket and will follow you and runs off to the couch, i dont know what to do, she only will eat if she is hand fed and will only come if you take her collar.. she is not a huge fan of men but im a girl and she is jumps and and everything im not sure what to do? she hates carpet and tile
